Patients that consent to this study will have certain details about their heart transplant recorded into their medical records information to be entered also into a registry sponsored by CareDx, Inc., Brisbane, CA (Sponsor). A registry is a collection of information about patients who have a particular disease or condition, or who receive a particular treatment, for research studies.
