The purpose of this research study is to evaluate if care coordination for women who screen positive for PPD can improve outcomes. The study will track how women are feeling for approximately 12 months. You are being asked to take part in this research study because you received a score of greater than or equal to ten (10) on the Edinburgh Postnatal Depression Scale (EPDS).
